Sjb medical are currently recruiting on behalf of our client, a clinical commissioning group (CCG). They are clinically-led statutory NHS body responsible for the planning and commissioning of health care services.

We are currently seeking experienced Nurses and Occupational Therapists to work as Band 6 CHC Nurse Assessors based in North East London.

The Role

You will join a multidisciplinary team of health professionals, conducting assessments of adults daily. This can involve covering various care groups such as people with mental health conditions, those needing palliative care, physically frail patients and adults with learning disabilities.

As an experienced practitioner, the role will enable you to utilise your clinical and evidenced-based knowledge and expertise to provide thorough assessments to determine eligibility of individuals to receive fully funded NHS continuing care or funded nursing care.

Main responsibilities

You will be involved in managing referrals to determine eligibility, identify and support the MDT in obtaining evidence to support the process and create robust documentation in accordance to the national framework and good practice guide. You will be providing clinical advice and ensuring a co-ordinated approach to care, that thoroughly reflects individual needs and considering the safe delivery of care.

Full time and part time roles are available with opportunities for flexible working.

Role overview:

Role requirements:

  • Relevant post registration experience and continuous professional development maintained
  • Good IT and typing skills, and the ability to write clear and accessible reports
  • Be able to demonstrate knowledge of current key policies and an understanding of the national framework for fully funded NHS continuing care.
  • Clinical knowledge gained within a broad range of healthcare settings
  • Strong attention to detail, a team player whilst being an independent thinker
